Best Mattress for Low Back Pain and Backaches: 2026 Guide

Back pain improves, in most cases, when the mattress provides the right support for the spine. It's not the only factor, but it's one of the few you can change overnight. This guide explains what firmness to choose, how it changes depending on your sleeping position, and when the problem is indeed the mattress. I'm writing it as I usually explain it over the phone.

The right firmness: neither hard nor soft

For years, the idea that a very hard mattress is better for your back was sold. Studies show the opposite. A trial published in The Lancet showed that people with chronic low back pain had less pain on a medium-firm mattress than on a very firm mattress.

The reason is simple. A mattress that is too hard doesn't follow the natural curve of the spine: the lumbar area is left without support. A mattress that is too soft allows the hips to sink and misaligns everything. Medium, or medium-firm, is the balance point for most people.

I say "most" on purpose. Your weight and sleeping position change the equation.

Your sleeping position determines half

If you sleep on your back

You need firm support in the lumbar area, without pressure points on the shoulders. Medium-firmness works well. Memory foam helps because it fills the gap between the lower back and the mattress. The Hisui mattress was designed for this profile: high resilience plus memory foam, firm, 26 cm.

If you sleep on your side

The shoulder and hip need to sink a little for the spine to stay straight. A mattress that is too firm creates pressure on the shoulder, and you wake up with a numb arm. If you sleep on your stomach

This is the position that puts the most strain on the lower back. If you can't change it, choose a firmer mattress so your stomach doesn't sink, and a low pillow. This reduces the arching of the lumbar area.

Pocket springs, memory foam, or hybrid?

The three respond differently to those with pain.

  • Pocket springs: each spring works independently. Good zone-by-zone support and excellent ventilation. Useful if you overheat at night or share a bed.
  • Memory foam: molds to the body and distributes pressure. Helps with pain points and for those who sleep on their back.

Weight matters more than you think

Heavier individuals sink more. If you are over 90 kg, a medium-firmness mattress may not be enough, and your lower back will lack support. In that case, I increase the firmness. The Samurai mattress — pocket springs plus high resilience, high firmness — provides the full support that a medium mattress cannot withstand.

Couples with very different weights are the most difficult case. In this situation, I prefer a medium-firm hybrid that accommodates both sides, like the Takumi.

Sometimes the problem is just the age of the mattress

A mattress loses support over time. If yours is more than 8 to 10 years old, or is already sagging in the middle, replacing it with a new medium-firmness one can resolve a lot of pain on its own—it's been studied. Before assuming you need the "firmest mattress on the market," ask yourself how long you've had your current one.

How to test without risk
